Actually, the first picture is real. John McCain really did speak with his microphone upside down. There are even YouTube videos about it: 01 – 02.

The second picture is from the Simpsons and the Old Man is shown as resembling McCain.

The third picture is an actual picture of John McCain making a weird face (which he is notorious for doing) and mimicking the Old Man from the Simpsons. For example, see What Is John McCain Sticking Out His Tongue For?.
